Deadlines

• Team Registration Form
including a $50 deposit--DEADLINE:  2 months prior to trip to United Methodist Ministries.  Make deposit check payable to United Methodist Ministries.

• Insurance Forms--DEADLINE:  4-6 weeks prior to trip.  The General Board of Global Ministries now requires that you go online to purchase accident insurance for your team.  This is a lengthy process, so start early.  Click here to go to the website.
  • Please remember that each person must complete their own form.
  • All accident insurance forms must be submitted in one batch and paid with one check.
  • Attach a cover sheet showing:  your team leader's name, address, phone, and e-mail; destination city; names and number of persons per each distinct set of dates of coverage.
  • Make Check Payable to:  General Board of Global Ministries, at $.80 per person per day, including days of departure and return.
  • NO cancellations.
  • Do not fax applications.
  • Address insurance form envelope to:  Mission Volunteers, Room 330, 475 Riverside Drive, New York, NY  10115
  • Team leader will be sent a copy of the letter to the accident insurance company for team coverage.
• Medical Release Forms--DEADLINE:  2 weeks prior to trip or upon arrival to United Methodist Ministries.
• Liability Release Forms--DEADLINE:  2 weeks prior to trip or upon arrival to United Methodist Ministries.
• Team Roster--DEADLINE:  2 weeks prior to trip to United Methodist Ministries.
• Final Payment--DEADLINE:  2 weeks prior to trip or upon arrival to United Methodist Ministries.
  • Make check for the mission fee payable to United Methodist Ministries. Note:  amount due for mission fee will be minus $50 deposit
  • Check for lodging will be separate and made directly to host church.  (Name of host church will be provided with the mission experience schedule.)
